Habilitation plan definition

Habilitation plan means an individualized, written plan for the client who is mentally retarded which includes measurable, time-specific objectives based on evaluations, observations, and other assessment data. The plan is based on the strengths and needs of the client and identifies specific staff responsibilities for implementation of the plan.
