HRSN Request definition

HRSN Request means a request from an HRSN Connector organization or individual made to an MCE for the purpose of requesting that the MCE perform an HRSN Eligibility Screening. An HRSN Request is comprised of, at minimum, the name and contact information of the individual being recommended and identification of the anticipated HRSN sService need. An HRSN Request may also include confirmation of OHP Medicaid enrollment or confirmation the individual is a Member enrolled in the MCE's CCO (or both), as well as any other information regarding the individual's potential HRSN Eligibility. The MCE shall be required to document its attempts to collect the information needed to determine eligibility.¶

Examples of HRSN Request in a sentence

  • Closed loop referral technology will provide a mechanism to conduct closed loop referrals and service provision between HRSN providers, the FFS TPC (described above) and CCOs. Data sharing between these entities may include general SDOH screenings, HRSN Request Form, person-centered service plan, and HRSN service referral and delivery status.

  • Transaction Request means a request from Seller to Buyer, in the form attached as Exhibit A hereto, to enter into a Transaction.

  • Drawdown Request means a notice substantially in the form set out in Schedule 3 (Drawdown Request).

  • Interconnection Request means a Generation Interconnection Request, a Transmission Interconnection Request and/or an IDR Transfer Agreement.

  • Extension Request means any Term Loan Extension Request or a Revolver Extension Request, as the case may be.

  • Written Request means a request made, in writing, by the Insured to us. All pronouns include either gender unless the context indicates otherwise.

  • L/C Request has the meaning specified in Section 2.4(b).

  • Borrowing Notice is defined in Section 2.8.

  • Notice of Revolving Borrowing shall have the meaning as set forth in Section 2.3.

  • Notice of Swing Line Borrowing has the meaning specified in Section 2.02(b).

  • Shelf Takedown Request shall have the meaning set forth in Section 3.2.5(a).

  • Arbitration Request shall have the meaning set forth in Section 7.3(a).
