Loss of Eligibility definition

Loss of Eligibility means loss of coverage resulting from divorce, death, termination of employment, a reduction in the number of hours of employment, or any loss of eligibility after a period that is measured based on any of those events. Loss of Eligibility shall not mean loss of coverage resulting from an individual’s failure to pay premiums on a timely basis or any termination of coverage for cause (such as making a fraudulent claim or an intentional misrepresentation of fact in connection with such coverage.)
Loss of Eligibility. “Active” SASMI Benefits (excluding Severance and Death Benefits) 1. Loss of Eligibility Based on Lack of Contributions. Your eligibility for Active SASMI benefits, other than the Severance Benefits, is lost if you have eighteen (18) consecutive months without SASMI contributions. 2. Loss of Eligibility Based on Use of Benefits without Recycling. Your eligibility for Active SASMI benefits, other than the Severance Benefits, is lost if you have received Supplemental Un/Underemployment Benefits in four (4) Stabilization Periods without recycling eligibility. The Periods in which you receive benefits do not have to be consecutive. The “Advance” Supplemental Unemployment Benefit is an advance of the Supplemental Un/Underemployment Benefit and will not be counted separately. However, if all of the Un/Underemployment Benefit you received in a particular Stabilization Period was paid as an “Advance” that counts as your Un/Underemployment Benefit. Once eligibility is re-established or recycled, the Supplemental Un/Underemployment check count resets to zero (“0”). However, any outstanding Welfare amount to be offset remains until it is erased as set out in II.B.

Examples of Loss of Eligibility in a sentence

  • A Medicaid or CHIP Special Enrollment Period begins on the day an employee’s or dependent’s coverage under Medicaid or CHIP terminates as a result of Loss of Eligibility.

  • A Special Enrollment Period occurs (i) after an employee’s or dependent’s coverage under another health plan terminated as a result of Loss of Eligibility, or (ii) after the employer providing such other health plan coverage terminated its contributions.

  • Create a LOSS_FROM label on the GTVSDAX form so that ZORHGPA, ZORZGPA and ZHRAPPS processes will begin counting the 1st/2nd Loss of Eligibility for HOPE and Zell Miller, beginning with your Fall 2011 term.

  • Disenrollment Due to Loss of Eligibility The participant will be disenrolled if he/she loses eligibility.

  • If the appeal status indicator is ‘Y,’ the Extension Credits cannot be zero.The Multiple Loss Indicator (Mult Loss) field is used to specify that the status represents multiple loss of eligibility.Note: The status code ‘NGPA’ (2nd Loss of Eligibility) should have the Mult Loss indicator checked, with no other indicators checked.The Appeal Extension Credits (App Ext) field is used to specify the total number of credits beyond a checkpoint that may be funded for a student, based upon an approved appeal.

  • In the event that a student loses HOPE twice(after Fall 2011), the ‘NGPA’ (2nd Loss of Eligibility) status code will be assigned to both the HOPE and Zell Miller funds on both eligibility blocks of the ZHAHOPE form.ZHAHOPE displays and allows for the maintenance of information related to HOPE/Zell Miller eligibility and HOPE/Zell Miller processing.

  • Termination due to Loss of Eligibility You and your Dependents must remain eligible to maintain your Group coverage.

  • These are outlined in the following sections which describe procedures that may be implemented when a student-athlete is placed in Phase 1, 2, 3, or 4, or Loss of Eligibility.

  • In the event of termination of this Contract or the loss of eligibility of an Enrollee, Delta Dental shall not be obligated to offer Services beyond the termination date of the Contract or the Loss of Eligibility Date or to pay for Services beyond such dates except for Single Procedures commenced while the Contract was in effect or the Enrollee was eligible.

  • Certificate of eligibility means a document issued by the bureau stating that the criminal record and all records of arrest, investigation, and detention associated with a case that is the subject of a petition for expungement is eligible for expungement.

  • Evidence of Eligibility means evidence of an individual's eligibility for training subsidised through the Skills First Program in accordance with the eligibility requirements set out in this VET Funding Contract and the Guidelines about Determining Student Eligibility and Supporting Evidence.

  • Retirement Eligibility means attainment of age 60 and completion of at least five (5) years of continuous service with the Company.

  • Loss of Speech means the disability in articulating any three of the four sounds which contribute to the speech such as the Labial sounds, the Alveololabial sounds, the Palatal sounds and the Velar sounds or total loss of vocal cord or damage of speech center in the brain resulting in Aphasia.

  • Eligibility means the decision as to whether an individual qualifies, under financial and nonfinancial requirements, to receive program benefits.

  • Class of Service means the various categories of service generally available to customers, such as business or residence.

  • Ineligibility means the Athlete or other Person is barred on account of an anti-doping rule violation for a specified period of time from participating in any Competition or other activity or funding as provided in Article 10.12.1;

  • Eligibility Waiting Period means the continuous length of time you must be in Active Employment in an eligible class to reach your Eligibility Date.

  • Loss of Use means the total and irrecoverable loss of function of an arm, hand, foot, leg or thumb and index finger of the same hand provided such loss of function is continuous for 12 consecutive months and such loss of function is thereafter determined on evidence satisfactory to Chubb Life to be permanent.

  • Eligibility Requirement means all eligibility requirements and other qualification requirements for a person to act in the applicable Corporate Trust Capacity under any Appointment as set forth in the related Corporate Trust Contract, including any required authorizations or licenses from the Federal National Mortgage Association, the Federal Home Loan Mortgage Corporation, the Government National Mortgage Association, the Federal Housing Administration, the Federal Home Loan Bank or the Department of Veterans Affairs.

  • list of eligible voters means the list referred to in rule 22.1, containing the information in rule 22.2;

  • Eligibility Service of an employee means the period or periods of service credited to him under the provisions of Article II for purposes of determining his eligibility to participate in the Plan as may be required under Article III or Article VI.

  • Level of service has the meaning set forth in Section 2.02(c).

  • Eligibility Requirements means, with respect to any Person, that such Person has at least $200,000,000 in capital/statutory surplus or shareholders’ equity (except with respect to a pension advisory firm or similar fiduciary) and at least $600,000,000 in total assets (in name or under management), and is regularly engaged in the business of making or owning commercial real estate loans (or interests therein), mezzanine loans (or interests therein) or commercial loans (or interests therein) similar to the Mortgage Loan.

  • Pregnancy means carrying a child, resulting childbirth, miscarriage and non-elective abortion. The Plan considers Pregnancy as a Sickness for the purpose of determining benefits.

  • Waiver eligibility span means the twelve-month period following either an individual's initial waiver enrollment date or a subsequent eligibility re- determination date.

  • Year of Eligibility Service means, for any Special Employee, a credit used to determine his eligibility to participate under the Plan, as further described in Section 2.2.

  • Fraud Loss Coverage Termination Date The point in time at which the Fraud Loss Coverage Amount is reduced to zero.

  • Small employer carrier means any carrier that offers health

  • Eligibility Determination means an approval or denial of eligibility and a renewal or termination of eligibility as set forth in OAR 410-200-0015;¶

  • Loss of Hearing means the total and complete loss of the ability to hear any sound as a result of a Covered Accident. The loss must be certified by a Legally Qualified Physician that the loss of hearing is permanent with no reasonable expectation of recovery.

  • Month of Service means a calendar month in which an employee is in receipt of full salary or wages in respect of the prescribed number of working hours in each working day in the month and includes a calendar month in which an employee is absent on special leave without pay not in excess of twenty (20) working days.

  • Loss of an Eye, means total and irrevocable loss of sight in that eye.

  • Retirement Eligible means that the Participant has either attained age 55 and completed ten (10) years of Service as an Employee or attained age 60 and completed five (5) years of Service as an Employee.

  • Employer/Client means the PSCA that signs the contract for the services/goods with the selected/qualified Contractor.

  • Eligibility Conditions means the eligibility conditions specified in the Act and the Rules including all the eligibilityconditions listed in Clause [5] of the Tender Document.
