Model of Care definition

Model of Care means the program designed by Health Plan and approved by CMS to meet the specialized needs of a Dual Eligible population that includes (i) an appropriate network of providers and specialists available through Health Plan’s D-SNP, and (ii) care management services, which include assessment, individualized plan of care and interdisciplinary team.
Model of Care means a CMS required written care management plan for Dual Eligible Medicare Advantage Special Needs Plans (DESNP) with eleven model of care elements and 4 State specific elements
Model of Care means the Model of Care and the Medical Termination of Pregnancy (has not exceeded nine (9) weeks of pregnancy) Care Pathway as published by the HSE and as may be amended from time to time in accordance with the requirements of Good Professional Practice.

  • Standard of Care has the meaning set forth in Section 14.1.
