New Entrant ACO definition

New Entrant ACO means a REACH ACO that is approved by CMS to participate in the Model as a New Entrant ACO prior to the Start Date or that has subsequently been approved by CMS to participate in the Model as a New Entrant ACO pursuant to Article VIII, unless the ACO has since accepted an offer pursuant to Section 5.03.B to participate in the Model as a Standard ACO. A New Entrant ACO qualifies for the alternative Beneficiary alignment minimums specified in Section 5.03.B and qualifies for the Performance Year Benchmark methodology specified in Section II of Appendix B.

Examples of New Entrant ACO in a sentence

  • If the ACO is a Standard ACO or a New Entrant ACO, the ACO may not be owned by an individual or entity that also owns another REACH ACO that is High Needs Population ACO that is participating in the Model and that operates in the same ACO Service Area, as defined in Section 5.04.

  • If the ACO is a High Needs Population ACO, the ACO may select to participate in the Model as a New Entrant ACO in advance of the ACO’s second Performance Year.

  • Further, prior to the start of both PY2025 and PY2026, the New Entrant ACO must have more than 3,000 beneficiaries aligned using claims-based alignment.

  • Additionally, of the 5,000 aligned beneficiaries a New Entrant ACO is required to have by PY2025, 3,000 or more must have been aligned via claims to show progress in establishing patient-provider relationships.

  • Conversely, New Entrant ACOs must have fewer than 3,000 claims-aligned beneficiaries for all three of these BYs; if a New Entrant ACO has at least 3,000 claims-aligned beneficiaries in at least one BY, they will be given the option to participate as a Standard ACO, provided they meet other eligibility criteria.

  • LOCKHEED XXXXXX Procurement Representative means a person authorized by LOCKHEED XXXXXX'x cognizant procurement organization to administer and/or execute this Contract.

  • Change Management means the add-on module to the Programs that enables engineers to define network changes through one or more configuration templates. Those network changes can be applied to multiple devices and executed/rolled back automatically. The Change Management module enables engineers to verify the impact of the changes across the network to help ensure a safer change process.

  • Radiation therapy simulation system means a radiographic or fluoroscopic x-ray system intended for localizing the volume to be exposed during radiation therapy and confirming the position and size of the therapeutic irradiation field.

The term does not include separate storm sewers in very discrete areas, such as individual buildings.

  • Exit Management means the obligations and rights of the Parties to ensure a smooth transition of the Framework from the Contractor to the Authority or any Replacement Contractor as set out in Clause 44 (Exit Management) and Schedule 8 (Exit Management).

  • Holding Tank Waste means any waste from holding tanks such as vessels, chemical toilets, campers, trailers, septic tanks, and vacuum-pump tank trucks.

  • Base Load Generation Resource means a Generation Capacity Resource that operates at least 90 percent of the hours that it is available to operate, as determined by the Office of the Interconnection in accordance with the PJM Manuals.

  • service delivery and budget implementation plan means a detailed plan approved by the mayor of a municipality in terms of section 53(1) (c) (ii) for implementing the municipality’s delivery of municipal services and its annual budget.

  • Drug therapy management means the review of a drug therapy regimen of a patient by one or more pharmacists for the purpose of evaluating and rendering advice to one or more practitioners regarding adjustment of the regimen.

  • Non-Administrator Substance Use Disorder Treatment Facility means a Substance Use Disorder Treatment Facility that does not meet the definition of an Administrator Substance Use Disorder Treatment Facility.

  • Quality Management Plan means the portion of the Project Development Plan providing the information requested in Section 4.3 of Exhibit B to the ITP.

  • Treatment Plant means the onshore plant to be constructed by the Joint Venturers for the liquefaction and treatment of natural gas and condensate as contemplated in the recitals to this Agreement.

  • Transboundary movement means any movement of hazardous wastes or other wastes from an area under the national jurisdiction of one State to or through an area under the national jurisdiction of another State or to or through an area not under the national jurisdiction of any State, provided at least two States are involved in the movement;
