The Performance Improvement Process may include: a requirement that the Hospital develop an Improvement Plan; or an amendment of the Hospital’s obligations as mutually agreed by the parties.
If, as part of a Performance Improvement Process, the Funder requires the Hospital to develop an Improvement Plan, the process for the development and management of the Improvement Plan is as follows: The Hospital will submit the Improvement Plan to the Funder within 30 Days of receiving the Funder’s request.
However, in accordance with County Policy, such employees should be placed on a formal Performance Improvement Plan with a time duration of ninety (90) days and receive a “Special Performance Evaluation”.
If the Contractor commits such a default, the Authority may elect to give a Performance Improvement Notice to the Contractor which shall specify the default in outline and the actions the Contractor needs to take with respect to remedying the default.
