If the employer determines to terminate or replace an existing HMO or Point of Service Plan, the Employer shall use its best efforts to obtain (or if the Plan is self administered, shall provide) reasonable transition rules permitting employees receiving ongoing medical treatment for an illness or injury to remain in the care of the treating physician[s] where that continuation is practicable and medically indicated.
