Adoption Assistance Program. The City will continue an Adoption Assistance Program for adoptions whereby employees in full-time non-seasonal classifications, with at least one year of continuous City service, may be eligible for adoption assistance up to three thousand five hundred dollars ($3,500) per adopted child. Adoption of a "special needs" child may provide for assistance up to five thousand dollars ($5,000). A "special needs" child is defined as a child qualified with special needs as described by each state agency under Title IV-E Program. Assistance will be on a reimbursement basis for specific adoption-related expenses. Only the following items will be considered for reimbursement:
(A) Licensed adoption agency fees (including fees for placement and parental counseling);
(B) State-required "pre-placement home study" and "post-placement supervision" program;
(C) Charges for temporary xxxxxx care before placement. The xxxxxx care must be provided by an approved or licensed agency and will be limited to thirty (30) days; and
(D) Charges for domestic transportation to obtain physical custody of the adoptive child. Transportation charges must be reasonable and be for both the adoptive parents and the adoptive child.

Effective as of the Transition Termination Date or such earlier date as Xxxxxxx-Xxxxx Squibb and Zimmer may mutually agree, Zimmer shall provide a Zimmer adoption assistance program to Active Zimmer Employees which is substantially similar in all material respects to the Xxxxxxx-Xxxxx Squibb Company Adoption Assistance Program. Effective as of the Transition Termination Date or such other date as Xxxxxxx-Xxxxx Squibb and Zimmer may mutually agree, the Xxxxxxx-Xxxxx Squibb Company Adoption Assistance Program shall cease to provide reimbursement to any Active Zimmer Employees and any and all outstanding approved reimbursements shall be made by Zimmer.
Adoption Assistance Program. Verigy Transferred Employees and other Verigy Employees shall cease active participation in the Agilent Adoption Assistance Program as of the applicable Agilent Participation Discontinuation Date. Notwithstanding the foregoing, in the event a Verigy Transferred Employee or other Verigy Transferred Employee is transferred to Verigy in accordance with Section 2.1 and such employee has an adoption pending under the Agilent Adoption Assistance Program, such employee shall continue active participation in the Agilent Adoption Assistance Program through such adoption and then shall cease active participation in such plan. Verigy shall not assume any Liabilities for benefits provided under the terms of the Agilent Adoption Assistance Program
Adoption Assistance Program. Within the first year of this Agreement, the University will create an Adoption Assistance Program for GSEs enrolled in PhD programs. The Program, which will operate on a reimbursement basis, will be designed to defray costs associated with adoption, up to $5,000 per adoption per family, with a maximum lifetime benefit of $10,000 per family.
Adoption Assistance Program. Discover Business Employees who have completed an adoption that is eligible under the Xxxxxx Xxxxxxx Adoption Assistance Program on or prior to the Distribution Date shall be entitled to benefits under that program. Discover shall reimburse Xxxxxx Xxxxxxx for the cost of providing such benefits in an amount determined consistently with the cost allocation method in effect prior to the Distribution Date. Discover Business Employees who complete an adoption after the Distribution Date shall not be eligible to receive benefits under the Xxxxxx Xxxxxxx adoption assistance program.
Adoption Assistance Program. Effective as of the Payroll Date, --------------------------- Agilent shall provide an Agilent adoption assistance program to Agilent Employees which has the Material Features of the HP adoption assistance program. As of the Payroll Date, the HP adoption assistance program shall cease to provide reimbursement to any Agilent Employees and any and all outstanding approved reimbursements shall be made by Agilent.

Financial assistance payments will be made after the adoption is finalized. A written request for reimbursement must be submitted to the Director of Human Resources along with the itemized bills. Written requests must be made within ninety (90) days after the adoption is finalized. Financial assistance payments will be made directly to the employee. The Department of Human Resources may request additional documentation regarding itemized bills.
Adoption Assistance Program. You remain eligible for the Adoption Assistance Program during your Payroll Continuation Period. Expenses that arise on or after your Termination Date are not covered.
Adoption Assistance Program. (AAP): A program of financial or medical assistance to facilitate the adoption of children who otherwise would remain in long-term xxxxxx care, as described in WIC Section 16115 through 16125.
