Affiliated Hospitals.
1. The Contractor shall have agreements with at least one hospital to support Contractor’s activities under this Contract and as further specified by EOHHS. Such hospital(s) shall be Affiliated Hospital(s).
2. The Contractor shall develop, implement, and maintain protocols with each Affiliated Hospital that support the coordination of Enrollees’ care, including transitions of care, as part of the Contractor’s Transitional Care Management program as described in Section 2.3.F.
Affiliated Hospitals. An Affiliated Hospital is a hospital that provides medical services to members of the public in the course of an approved medical or other professional health care clinical training program and that has an affiliation agreement with NYU to provide such training. In this document, Affiliated Hospitals include NYU Hospitals Center, Bellevue Hospital Center, NYU Hospital for Joint Diseases, Veterans Administration Hospital, Lenox Hill Hospital, Jamaica Hospital, Gouverneur Hospital, ▇▇▇▇▇▇▇▇ Hospital, Rockland Psychiatric Center, Rockland Children’s Psychiatric Center and Manhattan Psychiatric Center. Each Affiliated Hospital is a separate entity and cannot bind any others through its actions.
Affiliated Hospitals. The academic and research activities of UNLVSOM, including the teaching of medical students and the administration, supervision and training of residents and fellows of UNLVSOM, and the clinical care activities of UNLV Medicine, including medical directorships, call coverage and service agreements, often occur in hospitals that have entered into affiliation agreements with UNLVSOM and/or UNLV Medicine, respectively. UNLVSOM and UNLV Medicine shall exercise reasonable efforts to ensure that their respective affiliation agreements with these hospitals and/or health systems are coordinated. Any affiliation agreement with a hospital or health system approved by the UNLV Medicine board of directors shall be approved in advance by the UNLVSOM ▇▇▇▇. Any affiliation agreement (UNLVSOM or UNLV Medicine) shall include provisions that ensure adequate space is available to properly conduct patient care activities. UNLVSOM and UNLV Medicine shall work together to enforce those provisions with affiliated hospitals to, among other things: (1) protect their respective brands and financial viability; and (2) ensure faculty members and residents have proper and appropriate space to conduct their clinical, teaching and research activities with the support for required educational accreditation matters.

Payor agrees not to contract with any other contracting organization, preferred provider organization, health care facility, ancillary lab provider(s), outpatient surgery or radiology/imaging facility or physician(s) for the provision of Health Care Services in Jasper, Newton, Barton, Vernon, McDonald, and Cedar counties in Missouri, and Bourbon, Crawford, Cherokee, Labette and Montgomery counties in Kansas. This provision does not prohibit Payor from contracting for the following services: vision, dental, chiropractic care and outpatient pharmacy. Additional Missouri counties for CoxHealth Network: Barry, Christian, Dade, Dallas, Douglas, Greene, Hickory, Howell, Laclede, Lawrence, Ozark, Phelps, Polk, Stone, Taney, Texas, ▇▇▇▇▇▇▇, and ▇▇▇▇▇▇ Payor's Beneficiary Agreement or Plan includes financial incentives in the form of: (1) at least 20% points in co-insurance for using Participating Providers over non-Participating Providers; and (2) at least $1,000 difference in out-of-pocket maximum limits between Participating and non-Participating Providers. Payor agrees to provide ▇▇▇▇▇▇▇ with a copy of the portion of the Beneficiary Agreement or Plan documents (and any relevant updates) describing the financial incentives. Payor agrees to communicate to its Beneficiaries that it has arranged for discounted services with ▇▇▇▇▇▇▇ Health System hospitals and physicians.
