Application for a Lane Change. To apply for a lane change, please submit the ‘Lane Change Application Form’ (available on the HR webpage) and a copy of your transcript(s) and/or Board Certificates to Human Resources. Applications shall not be considered valid or effective unless accompanied by an official transcript bearing evidence of the completed credits or degree program. Electronic transcripts are deemed official only if they are received from a third-party administrator.
