APPLYING FOR A SERVICE. 2.1 How to make an Application for a Service You can make an Application for supply of a Service by Us either by: Emailing Us a description of Your service requirements; Completing and sending Us an online application form (located on Our website or through Our website portal); or Completing and returning paperwork that We have provided to You. We will assess each Application to ensure that it is technically and operationally sound and consider Your eligibility for the Services. We do not guarantee that the Service You request in an Application will be available to You. If We accept Your Application and provide You with a SCN in accordance with clause 2.3, We will start providing the Service to You on the date specified in the SCN unless both parties have agreed another date that We have notified to You by email. From time to time We may include Special Conditions in a SCN, and the Special Conditions apply only to the Services stated in that SCN. Sometimes Your Application to Us might include a document which We have prepared for You, which sets out Our proposal to You for the Service and this might occur when You have complex communication needs. Such a document whether expressly or impliedly approved by You will be an Application under this Contract. Under this Contract, You are able to make more than one Application and therefore buy multiple Services. Each Service will be for the relevant Service Term.

  • Fee-for-Service In providing its services and operations, Contractor will maintain compliance with requirements of the AOD Policy and Procedure Manual including additions and revisions, incorporated by reference herein. The maximum length of stay is set by the specific funding source and an extension beyond a 90 day period may be granted only by written approval from the Alcohol and Other Drug Services (AOD) Administrator, pursuant to a Contractor’s written request outlining and justifying the client’s clinical need. Reimbursement will be approved only for clients who referred through the formal referral process outlined in the AOD Policy and Procedure Manual.

  • Terms of Service In a Cloud environment, Red Hat’s Software Subscriptions may provide you with access to the Software and associated maintenance (updates, upgrades, corrections, security advisories and bug fixes), if and when available, in the form of software images intended to be deployed as virtual instances. Payments to Red Hat for Software Subscriptions do not include any fees that may be due to the Vendor for the Vendor’s Cloud services.
