Beneficiary Service Records. The PIHP shall establish and maintain a comprehensive individual service record system consistent with the provisions of MSA Policy Bulletins, and appropriate state and federal statutes. The PIHP shall maintain in a legible manner, via hard copy or electronic storage/imaging, recipient service records necessary to fully disclose and document the quantity, quality, appropriateness, and timeliness of services provided. The records shall be retained for a minimum period of seven (7) years from the date of service or termination of service for any reason. This requirement must be extended to all of the PIHP's provider agencies.

The PIHP shall maintain in a legible manner, via hard copy or electronic storage/imaging, recipient service records necessary to fully disclose and document the quantity, quality, appropriateness, and timeliness of services provided. The records shall be retained according to the retention schedules in place by the Department of Management and Budget (DTMB) General Schedule #20 at: xxxx://xxxxxxxx.xxx/dmb/0,4568,0-000-0000_21738_31548- 56101--,00.html. This requirement must be extended to all of the PIHP's provider agencies.
