Common use of CDL Physicals Clause in Contracts

CDL Physicals. The City of Xxxxxx agrees to pay for CDL physicals that are required for certain positions within the Unit. The City will pay the fee for the physical in full if it is obtained by the City’s provider, currently Occupational Medicine Associates, 000 X. Xxxxxx Xxx, Xxxxxxx, XX 00000. CDL physicals are required every two years in order to stay current and the City will pay this fee every two years. It is the responsibility of the employee to schedule the physical and to have it billed back to the city. It is also the responsibility of the employee to provide proof of fitness for duty per the results of this physical. This was agreed to in the last negotiations for the 2009 contract. The City of Xxxxxx agrees to pay for CDL physicals that are required for certain positions within the Unit. The City will pay the fee for the physical in full if it is obtained by the City’s provider, currently MultiCare Centers for Occupational Medicine. Should an employee choose to seek another provider to perform the required CDL Physical, the provider must be state certified. The City will reimburse the employee for the cost of the required CDL Physical examination up to the cost of a CDL Physical performed by the City’s current provider. The employee will be responsible for any amount above the rate charged by the City’s current provider. It is the responsibility of the employee to schedule the physical and to have it billed back to the city or the employee will provide a receipt if the CDL Physical examination is performed by a certified physician other than the City’s current provider. It is also the responsibility of the employee to provide proof of fitness for duty per the results of this physical.
