Certification Decision. The assessment team hands over the audit records (audit report, list of completed VBM, audit checklist, attendance list) which serve as the basis for the certification committee's decision. A positive certification decision can only be made if all certification criteria are met and any critical non-conformity has been successfully resolved. The certification committee shall comply with the requirements from FB_2-02-08_Certification_Committee_TSI. No member of the evaluation team shall be a member of the certification committee and vice versa. The certification body (the actual authorized signatories) make the decision based on the opinions of the certification committee. A certificate is to be issued for a period of up to 5 years. The assessment of whether there is a positive evaluation result is the sole responsibility of the CAB.

Completion of 100% of the remaining goals/objectives for the program with at least 50% of the objectives having a status as achieved. All objectives must have a minimum status of satisfactory progress. • All evaluations must be completed in accordance with ASHP Accreditation Standard as outlined in the Resident Agreement • Completion of pharmacokinetic certification by October 31 of residency year • Completion of inpatient anticoagulation certification by October 31 of residency year. • Completion of major project, which includes meeting all project deadlines and submission of written manuscript of completed project to the RAC. • Assigned projects (MUE, drug review, class review, and management project) have been completed • Presentation of at least one major CME program. • Completion of minimum staffing requirements as described in section 2.7 • Adherence to the organization’s attendance standards and as described in this document under Exhibit A.

  • CERTIFICATION CLAUSES The CONTRACTOR CERTIFICATION CLAUSES contained in the document CCC 04/2017 are hereby incorporated by reference and made a part of this Agreement by this reference as if attached hereto.

  • AS9100 Certification ‌ AS9100 Certification, specifies requirements for a quality management system to demonstrate the Contractor’s ability to consistently meet the customer requirements as well as statutory and regulatory requirements for the aerospace industry. An AS9100 Certification, is not mandatory; however, Contractors who desire to compete for work within the aerospace industry are encouraged to have AS9100 Certification, during the entire term of OASIS. The Contractor shall notify the OASIS CO, in writing, if there are any changes in the status of their AS9100 Certification, and provide the reasons for the change and copies of audits from an AS9100 Certification Body, as applicable. If only part of a Contractor’s organization is AS9100, certified, the Contractor shall make the distinction between which business units or sites and geographic locations have been certified.

  • Tax Certification If Contractor is a taxable entity as defined by Chapter 171, Texas Tax Code, then Contractor certifies it is not currently delinquent in the payment of any taxes due under Chapter 171, Contractor is exempt from the payment of those taxes, or Contractor is an out‑of‑state taxable entity that is not subject to those taxes, whichever is applicable.

  • Professional Certification The Superintendent shall at all times during the term of this Contract, and any renewal or extension thereof, hold and maintain a valid certificate required of a superintendent by the State of Texas and issued by the State Board for Educator Certification or the Texas Education Agency and any other certificates required by law.
