Co-Payment. A cost-sharing arrangement in which a covered person pays a specific charge for a specified service. This amount is paid at the time services are rendered.
Co-Payment. Shall be defined as the predetermined (flat) dollar amount that a Covered Person must pay for certain Covered Services as stated in this Agreement and after the Deductible, when applicable, has been met.
Co-Payment. The fixed amount certain CHIP Members pay for a covered health care service. The amount may vary based on the health care service being provided.
Co-Payment. A co-payment is a cost-sharing requirement under a health insurance policy that provides that the policyholder/insured will bear a specified percentage of the admissible claim amount. A co-payment does not reduce the Sum Insured.
Co-Payment. The Insured Person will bear a predetermined percentage of the admissible claim amounts subject to the Co-payment option chosen by You irrespective of the Age of the Insured Person and the number of claims made. Co-payment will not apply to any claim under section 3.6 (Second Medical Opinion), Section 4.2 (Health Checkup) and Section 5.1 (Hospital Cash).
Co-Payment. We will offer a Co-Payment as specified in the Policy Schedule or Certificate of Insurance that shall be applicable on the payable claims under the Policy.
Co-Payment. Fixed fee paid pursuant to this Agreement to a Provider by Enrollee at time of provision of behavioral health services, which are in addition to the premiums paid by the Employer/Trust. Such fees may be a specific dollar amount or a percentage of total fees, depending on the type of services provided. The EAP has $0.00 copay.
Co-Payment a. The CCIS assesses each family a weekly co-payment which is paid on the first service day of the week and each week thereafter. Parents/Caretakers are also required to pay an equivalent advance co-payment to the provider, which is refunded to the parent/caretaker upon termination of the child’s enrollment.
b. The co-payment covers all the days of the week for which the CCIS authorizes child care.
c. The full co-payment amount is due to the provider regardless of the number of days of care a child receives in a week.
d. It is the provider’s responsibility to collect the co-payment on a weekly basis and report any unpaid co-payment to the CCIS. The co-payment is delinquent if it is not paid by the last day of the service week. The provider must report a delinquent co- payment to the CCIS on the business day following the day the co-payment becomes delinquent.
e. When the parent/caretaker incurs a delinquent payment, and the parent/caretaker makes a payment to the provider, the provider must apply the payment toward the co-payment due for the current week of care. The provider must then apply any remainder of the payment toward the delinquent amount. The provider must notify the CCIS when the delinquent co-payment is resolved.
Co-Payment. Parent/Participant payment made directly to the child care provider when that provider’s child care fee exceeds the Regional Market Rate Ceiling .
Co-Payment. You are required to collect the assessedparent fee/co-payment from the parent.
a. The amount of the co-payment is listed in the child’s Portchaildlcaree cnerrtifoicaltel. Ifmthee ncot- / payment changes, we will notify you in writing . Only co-payment changes from ELCFH are valid.
b. Co-payments are automatically deducted from your monthly reimbursement.
c. You must give the parent a receipt for each co-payment collected and retain the receipt records. Upon request, you must provide a current accounting and copy of co -payment receipt records to ELCFH. We use this documentation to ensure parents meet their co-payment obligations.
