Dental Plan - M. S.A. Dental Plan Effective April 21, 1994 increase the per person annual maximum on Basic services to one thousand dollars ($1,000). Effective May 1, 1991 the Dental Plan to provide the maximum per year for basic services of $700.00. The increase shall only apply to eligible expenses incurred after the above date. Effective May 1, 1991 the combined lifetime maximum for Restorative and Orthodontic Services is $4,000. The increase shall only apply to eligible expenses incurred after the date of the ratification. Effective April 21, 1992, there will be a provision for an annual re-instatement of up to $500 at the beginning of each calendar year, if required to restore such lifetime maximum to the level of $4,000. This change shall only apply to eligible expenses incurred after the above date. Effective May 1, 1991 increase the existing "Restorative" coverage from fifty (50) percent of eligible expenses to seventy-five (75%) percent of eligible expenses, for all such expenses incurred after the above date.

  • Dental Plan (a) The Employer shall pay the monthly premium for employees entitled to coverage under a mutually acceptable plan which provides:

  • Group Dental Plan Upon proper application, Benefit Eligible Employees will be enrolled, along with their eligible dependents, in the Employer's group dental plan and will be provided with the coverages specified therein.
