Group Dental Plan. Upon proper application, Benefit Eligible Employees will be enrolled, along with their eligible dependents, in the Employer's group dental plan and will be provided with the coverages specified therein. The Employer will pay the required premiums for the plan on a single/family composite basis.
Group Dental Plan. 32-12 The Group Dental Plan shall be as provided by the University in the same manner and to the same extent as provided to University employees not represented by a union within the Benefits Program. Employees have a choice of three dental plan options. During the term of this Agreement, no less than the University of Michigan Dental Plan, Option I schedule of benefits in effect at the execution date of this Agreement will be provided and maintained. In the event of any changes in the benefits, the Union will be notified prior to the effective date of change. The University contribution toward dental plan coverage will be provided in the same manner and to the same extent as provided to University employees not represented by a union.
Group Dental Plan. For employees covered by this Agreement, group dental coverage is available. A plan which provides certain benefits to age sixty-five (65) is available to permanent full-time employees and their dependents. The City will pay the employee’s premium.
Group Dental Plan. The Group Dental Plan shall be as provided by the Employer in the same manner and to the same extent as provided to tenure- track and tenured faculty. Employees shall have a choice of three (3) dental plan options. The University contribution toward dental plan coverage shall be provided in the same manner and to the same extent as provided to tenure-track and tenured faculty. The Employer will automatically enroll Employees who do not elect to opt out or enroll in a dental insurance plan within 30 days of the eligibility date in the University of Michigan Dental Option I plan with Employee only coverage.
Group Dental Plan. For employees covered by this Agreement, group dental coverage is available. A plan which provides certain benefits is available to permanent full-time employees and their dependents. The City will pay the employee’s dental premium. The employee will pay for dependent coverage if desired.
Group Dental Plan. 20.3.1 The premiums for the dental plan will be paid seventy percent (70%) by the Library and thirty percent (30%) by the employees whose contributions shall be made by payroll deduction. Basic dental service (Plan A) paying for eighty percent (80%) of the approved schedule of fees.
20.3.2 Restorative dental service (Plan B) paying for fifty percent (50%) of the approved schedule of fees.
20.3.3 Effective January 1, 2014, Orthodontics (Plan C) – Employer pays fifty percent (50%) of approved schedule of fees. The orthodontic lifetime maximum per child shall be two thousand dollars ($2,000.00). Coverage shall only apply to dependent children.
Group Dental Plan. After one year of seniority. 100% of the premium cost for a dental plan with coverage equivalent to that presently provided for eligible employees and their eligible dependents.
Group Dental Plan. The Employer shall pay the premium cost of a group dental plan for those employees who regularly work an average of twenty-four (24) hours or more per week in the bargaining unit. An employee who elects dependent coverage will pay for group coverage, including the medical and dental and vision coverage, through payroll deduction. See Section
13.3.1 for premium costs. The dental deductible shall be fifty dollars ($50) per year per person with an aggregate deductible of one hundred fifty dollars ($150) per family. Maximum paid dental benefit shall be one thousand five hundred dollars ($1,500) per calendar year.
Group Dental Plan. The current dental plan shall continue in effect during the term of this Agreement, provided the entity providing such plan so allows. For each teacher covered by this Agreement, the Board shall pay toward the premium cost of the plan which the teacher has properly enrolled in, during each year of this Agreement, a maximum amount for each plan as set forth in the table below. Additional costs shall be shared equally between the board and the individual teacher.
Group Dental Plan. 295 The Group Dental Plan shall be as provided by the University in the same manner and to the same extent as provided to the University employees not represented by a Union. Employees have a choice of three dental plan options. During the term of this Agreement no less than the University of Michigan Dental Plan, Option I schedule of benefits in effect at the execution date of this Agreement will be provided and maintained. In the event of any changes in the benefits, the Union will be notified prior to the effective date of change. 296 The University contribution toward dental plan coverage will be provided in the same manner and to the same extent as provided to University employees not represented by a Union. The University will automatically enroll employees in the University of Michigan Dental Plan, Option I after one year of continuous service as provided to University employees not represented by a Union. 297 No matter concerning the Group Dental Plan shall be subject to the Grievance and Arbitration Procedures, except for questions concerning compliance with the specific provisions of this Article, and whether or not the employee has coverage in accordance with terms of the Plan. 298 If, during the term of this Agreement, a federal or state law is enacted which requires the payment of taxes or premiums to either the federal or state government or another entity for dental benefits for employees, the University may make such adjustments in the schedules of benefits provided by this Article to avoid duplication of benefits. In addition, any such taxes or premiums paid by the University shall be included in the total dollar limitation provided in this Article.
