Educational Assistant Assessment Form. Employees who receive notice that their position is cut or reduced are required to complete the educational assistant Assessment Form. This form must be completed and returned to the Human Resource Department no later than June 30 of each year. Failure to provide a completed assessment form to the Human Resource Department by the June 30 deadline will disqualify the employee for recall/placement until the assessment has been submitted. The District will not be required to change any previous placements as a result of an employee failing to meet these requirements.

Educational Assistants will receive the Assessment Form at the time they receive notice of a position cut or reduction and will have two weeks from receiving the assessment form to return it to Human Resources.
