EHC. (i) Drugs -*$150.00 maximum benefit/covered person.
EHC. The Employer agrees to provide to all post-probationary full-time employees the following coverage. The Employer agrees to pay 75% of the premiums for the following plans. There is a maximum of twenty-five thousand dollars ($25,000.00) in a lifetime in combination with hospital and drugs. (a) Extended Health (EHC) No annual deductible Available only by prescription. No survivor benefits. Vision care coverage of three hundred ($300) dollars in two consecutive years. Paid eighty (80%) percent on the first twenty-five hundred dollars ($2,500.00) per calendar year. (b) Life Insurance $15,000.00 100% Employer paid premium No benefit will be reduced as a result of changes made by the employer to the benefit provider, or due to benefit changes.
EHC. In any event, once an employee reaches age 70 and she continues to be employed she shall automatically be placed on the percentage-in-lieu as per Article 21.04 for all items now included in the payment.
EHC. (i) Drug car provided, $1 .00 co-payment per prescription. Cover prescription drugs which by law must be prescribed by a licensed physician. Generic substitution unless specifically prescribed otherwise by the doctor. Dispensing fee cap of seven dollars and fifty cents ($7.50) per prescription. (No annual deductible or lifetime maximum for drugs.) (ii) Vision - no benefit during first six months. (iii) Hearing - no benefit during first six months.
EHC. The Employer agrees to contribute 75% of the billed premium towards coverage of eligible full-time employees under the current Extended Health Care Plan. The Extended Health Care Plan shall provide for a Hearing Aide benefit with a maximum of $300.00 per insured individual in each five (5) year period.
EHC. There will be an increase to $350 per calendar year for the following: Osteopathy, Podiatrist/Chiropodist (combined), Psychologist/Social Worker (combined) and Speech Language Pathologist. Extended health care coverage will be $350 per calendar year for the following: • Osteopathy • Pod/Chiro (combined)

EHC. LLC shall not be liable to Contractor for, and shall not reimburse Contractor for any expenses paid or incurred by Contractor as a result of the work or of this Agreement. All compensation must be invoiced.
