Common use of Electronic Professional Responsibility Workload Report Forms Clause in Contracts

Electronic Professional Responsibility Workload Report Forms. (a) The parties agree to use the electronic version of the ONA/OHA Professional Responsibility Workload Report Form (PRWRF) at Appendix 6 of the Hospital Central Agreement. (b) The parties agree that hard copies of the electronic PRWRF are valid for purposes of Article 8 of the Hospital Central Agreement. (c) Electronic PRWRFs may be sent, via email, to the applicable manager or designate. (d) The electronic signature of the Union Executive or Labour Relations Officer will be accepted as the original signature. (e) The Union undertakes to get a copy of the electronic version signed by the employee(s). (f) The parties agree to not use or rely upon any preliminary arguments related to the use of the electronic version should a PRWRF proceed to an Independent Assessment Committee as per Article
