Electronic Professional Responsibility Workload Report Forms. (a) The parties agree to use the electronic version of the ONA/OHA Professional Responsibility Workload Report Form (PRWRF) at Appendix 6 of the Hospital Central Agreement. (b) The parties agree that hard copies of the electronic PRWRF are valid for purposes of Article 8 of the Hospital Central Agreement. (c) Electronic PRWRF’s may be sent, via email, to the applicable Manager or designate, copying all members submitting the PRWRF’s. (d) The electronic signature of the Union Representative, Labour Relations Officer and the Manager will be accepted as the original signature. (e) The Union undertakes to get a copy of the electronic version signed by the employee(s). (f) The parties agree to not use or rely upon any preliminary arguments related to the use of the electronic version should a PRWRF proceed to an Independent Assessment Committee as per Article 8.01. (g) Should the Union wish to move to a different form of submission, this change will not result in any additional cost to the Hospital.
