Employee Contact Information. Employees are required to notify the Company of their current address and telephone number, and encouraged to provide an email address. The Company shall be entitled to rely upon the last address and telephone number furnished by the employee for all purposes. Employees shall have the responsibility of forwarding a completed change of address form to the Human Resource Department if there are any changes in address, telephone number or emergency contact information. The company shall make available a two (2) part form for this purpose. One (1) copy will be for the Human Resources Department and one (1) copy for the employee’s records. The Company agrees to furnish the Local Union with a confidential listing of name, address, and telephone number upon request.
