Common use of Employee List Clause in Contracts

Employee List. Contractor shall submit to the Department within thirty (30) days of the execution of this Agreement and upon request a list of certain employees, which shall include the Executive Director, Chief Financial Officer, Chief Operating Officer, and/or the functional equivalent of such positions, and key employees (as the phrase “key employee” is defined in the Instructions to IRS Form 990). For each listed employee, Contractor shall provide the current total compensation (including all benefits), all sources of the employee’s total compensation, whether from this Agreement or another City, State, Federal or private source, and the dollar amount of compensation from each such source.

Contractor shall submit to the Department within thirty (30) days of the execution of this Agreement and upon request at the beginning of each new fiscal year a list of certain employees, which shall include the Executive Director, Chief Financial Officer, Chief Operating Officer, and/or or the functional equivalent of such positions, and key employees (as the phrase “key employee” is defined senior financial and programmatic supervisory personnel involved directly or indirectly in the Instructions to IRS Form 990)performance of this Agreement. For each listed employee, Contractor shall provide the current total compensation (including all benefits), all sources of the employee’s total compensation, whether from this Agreement contract or another City, State, Federal or private source, and the dollar amount of compensation from each such source.
