EMPLOYEE SIGNATURES. 11.2.1 No information on disciplinary actions except notices of discharge shall be placed in the employee's official personnel file that does not bear the signature of the employee. The employee shall be required to sign ' material to be placed in his/her official personnel file provided the following disclaimer is attached: Employee's signature confirms only that management has discussed and given a copy of this material to the employee. The employee's signature does not indicate agreement or disagreement with the contents of this material. 11.2.2 If an employee is not available within five (5) working days or refuses to sign the material, the Employer may place the material in the file. 11.2.3 No disciplinary material shall be placed in an employee's file unless the employee has had an opportunity to sign it and has received a copy. If presented with a disciplinary notice, an Employee must sign to acknowledge that they have received the notice, regardless of whether or not the Employee agrees with the content. An employee has the right to attach her/his own views to any disciplinary record in her/his own file.
