Employees Eligible to Participate. Any regular employee of the Company or any of its subsidiaries who is in the employ of the Company on one or more offering dates is eligible to participate in the Plan, except (a) employees whose customary employment is less than 20 hours per week, and (b) employees whose customary employment is for not more than five months in any calendar year.

The Employer hereby requests that the Board of Trustees of the Trust permit the Employer to participate in the Trust’s Substance Abuse Testing and Treatment Policy and Program, including the Employee Assistance Program (“Program”) on behalf of certain non-bargaining unit employees or positions (“Associate Employees”), as defined below: (Please attach additional sheet, if necessary.)
