Enrollment and Service Delivery a. Following initial assessment, CONTRACTOR will provide staff assistance to individuals in completing and signing an initial application for services and direct youth in next steps for enrollment. CONTRACTOR will, if appropriate, enroll youth and provide services to youth clients at an AJCC or other WDB designated location. CONTRACTOR will enter all enrolled clients’ data into CITY’s CalJOBS system within three (3) business days of enrollment. Any client whose data is not uploaded into CITY’s CalJOBS system will not be considered as enrolled which may result in disallowed costs.

CONTRACTOR will provide staffing to meet the needs of this AGREEMENT at the San Xxxx AJCC or another location as designated by CITY. Case managers shall be available to clients Monday through Friday, except mutually agreed upon holidays, to assist them with their goals. Youth and CONTRACTOR case managers will work together to determine the appropriate services.

Contractor will enter all enrolled Youth information into CalJOBS. Enrollment must occur within five (5) days of the Youth’s initial orientation. Any Youth participant whose information is not uploaded to CalJOBS may be considered not enrolled, resulting in disallowed costs.
