Enrollment Requirements. You must maintain with Blue Cross and Blue Shield a current and updated listing of covered employees. You will be responsible for all claims costs and expenses associated with failure to maintain an accurate and current listing with Blue Cross and Blue Shield, unless such claims costs and expenses are due to an error on Blue Cross and Blue Shield’s part. In order to maintain health care coverage with Blue Cross and Blue Shield, an employee must meet the written eligibility requirements (such as length of service, active employment and number of hours worked) you impose as long as they do not conflict with Blue Cross and Blue Shield’s eligibility requirements. An eligible employee as defined by Blue Cross and Blue Shield means: • A permanent full-time employee regularly working 30 hours or more each week at the employer’s usual place(s) of business and who is paid a salary or wage in accordance with state and federal wage requirements; or • A permanent part-time employee regularly working at least 20 hours but less than 30 hours each week at the employer’s usual place(s) of business and who is paid a salary or wage in accordance with state and federal wage requirements; or • A disabled permanent full-time or part-time employee who is actively working despite the disability (including one who is engaged in a trial work period) and a disabled employee who is not actively working but whom the employer treats as an employee; or • A former employee (or a former covered dependent of the employee of the group) who qualifies for continued group coverage under federal or state law, but only if the employer maintains Blue Cross and Blue Shield group coverage for permanent full-time employees as defined in (a) above; or • A retired employee of the employer. Newly hired employees who are eligible for group benefits can enroll in the benefits plan according to your eligibility requirements for coverage, provided that your requirements comply with Blue Cross and Blue Shield’s eligibility and enrollment requirements. The effective date of an eligible employee’s (or his or her dependent’s) membership in the benefits plan may be the Member’s initial eligibility date or your subsequent anniversary/renewal date, as long as: (a) Blue Cross and Blue Shield receives your written notice no later than 30 days after the Member’s enrollment notification period applicable to membership modifications (as described in the Subscriber Certificate for your benefits plan); and (b) you pay ...
Enrollment Requirements. As a condition of participation, each selected Employee shall complete, execute, and return to the Plan Administrator all form(s) required by the Plan Administrator and within the time specified by the Plan Administrator. In addition, the Plan Administrator shall establish such other enrollment requirements as it determines necessary or advisable.

Enrollment Requirements. The Contractor agrees to conduct Enrollment of Eligible Persons in accordance with the policies and procedures set forth in Appendix H of this Agreement, which is hereby made a part of this Agreement as if set forth fully herein.
Enrollment Requirements. Rooms are assigned to students enrolled at the university for the fall, spring, and summer semesters. All residents must be registered for classes prior to the semester before they will be permitted to check-in to their campus housing assignment. Summer semester residents not taking classes must be pre-enrolled for the ensuing fall semester if they wish to obtain a contract during summer semester. If a student is currently living on campus for summer semester and has a 2021-2022 housing contract, they will not be permitted to move to their Fall housing assignment if they are not registered for Fall semester classes. If a student is currently living on campus for summer semester and planning to live in the same unit for 2021-2022, they will need to check-out July 29, 2021 if not registered for Fall semester classes. If a student is not registered for classes, the student will not be permitted to move into their Fall 2021 or Spring 2022 campus housing assignment. The housing assignment will be canceled, they will forfeit their $150 prepayment and a registration hold will be placed on their account. Should the student choose to enroll at Xxxxxx State University anytime during the 2021-2022 academic year, they will be obligated to live on campus. In the event that the student has moved in early for Fall 2021, they must move out of their housing assignment no later than August 18, 2021 and return keys to Residence Life & Housing. Once their keys are returned or the locks are changed, their housing assignment will be canceled, their $150 prepayment will be forfeited and a registration hold will be placed on their account. Should the student choose to enroll at Xxxxxx State University anytime during the 2021-2022 academic year, they will be obligated to live on campus. Fall 2021 residents whose Spring 2022 registration is canceled/not registered for classes must move out and return keys to Residence Life & Housing no later than January 3, 2022. Once keys are returned and/or locks are changed, their housing assignment will be canceled and a registration hold will be placed on their account. Should the student choose to enroll at Xxxxxx State University anytime during the 2020-2021 academic year, they will be obligated to live on campus.
Enrollment Requirements. All full-time unit members shall be required to enroll in a District-provided health insurance, dental insurance and vision care plan. Part-time unit members working a minimum of fifty percent (50%) but less than one hundred percent (100%) of the school year, may participate in the District provided group medical, dental and vision insurance and shall receive a prorated share of the health and welfare premium contribution.

The eligible enrollment age of Pre-K and Kindergarten students will be determined by the policy of the Diocese of Little Rock and the State of Arkansas for the current school year. A birth certificate is required for initial enrollment of new students. If a child is a Catholic, a baptismal certificate must also be presented at the time of registration.

The provider is a physician in the field of General Practice, Family Practice, General Pediatrics, General Internal Medicine or Obstetrics/Gynecology and operates a primary care mode of practice OR a physician in any specialty who agrees to the responsibilities and requirements of a Care Select Program PMP, as further outlined in this Addendum and CMO policies.
Enrollment Requirements. 2.1 Agree to not drink alcohol or use any illicit drugs while living at Peggy’s Promise. Also agree to not have any alcohol, illegal drugs, or drug paraphernalia on the property at any time.
2.2 Ability to pass a drug and alcohol test and agree to submit to weekly and random testing.
2.3 Agree to continue working a 12-Step program by attending a minimum of 4 AA/NA/CA meetings a week, in addition to a Accountability House Meeting and will work a program of recovery with a sponsor. Must find a twelve-step sponsor within 7 days (a temporary sponsor is not acceptable). Once an individual has worked through Steps 1 – 9 with a sponsor their meeting requirements are based upon individual programs.
2.4 Respect the rights, views and property of other enrollees, and get along with others.
2.5 Agree to contribute to the cleanliness of the house by doing your chores and picking up after yourself.
2.6 Agree to abide by all the house rules.
2.7 Ability and agreement to pay the Peggy’s Promise member Fees.
2.8 Agree to take prescription medications as prescribed and be in good medical health, free of communicable disease that put others at risk of contraction through casual contact. House management must approve all prescription medications. Prescription medication cannot be on the prohibited list.
2.9 All residents are required to obtain gainful employment or 20 hours of volunteer work within 14 days of enrollment.
