Case Managers. The Board will allow each case manager the equivalent of two (2) days each year to be taken in no less than one-half (1/2) day increments, to be relieved of regular duties for the purpose of constructing IEPs. The Board will provide a substitute for the case manager who will report to his or her school for these days.
Case Managers. The terms and conditions of employment applicable to Case Managers are contained in this Article.

Case Managers. The case manager responsible for the preparation of documents for Individual Education Plans will be granted two (2) full days of release time (1 each semester) to prepare for annual reviews, summarize progress monitoring data, or prepare alternative assessment materials. The member must use the release time to work in their assigned building or elsewhere in the district with prior approval from the building administrator.
Case Managers. 27.1.1.1. All Staff currently carrying out responsibilities and duties of a Case Manager on a full-time basis shall be titled as such and be bargaining unit members.
27.1.1.2. The school shall exercise best efforts to maintain at least one (1) full-time Case Manager.
27.1.1.3. A caseload of 125 students shall trigger review by the school administration to consider the need for additional case managers.
27.1.1.4. Case Managers shall not be assigned lunch/recess duty.
27.1.1.5. Except in cases of emergency, for LBS1 certified educators, Case Managers shall not be assigned as substitute teachers.
27.1.1.6. Case managers may volunteer to work additional days beyond the work year otherwise specified in this Agreement. These additional workdays may be scheduled in increments (i.e., the additional days are not worked consecutively) as operationally necessary at the start or end of the summer break period. These additional workdays shall be scheduled in advance in collaboration with the Administration. Case managers shall be paid at their effective hourly rate for all hours worked beyond the school year based upon their annual salary.
Case Managers. Contractor shall provide qualified case managers who will: Conduct a comprehensive psychosocial assessment and develop an ISP to address all of the supportive services needs of the AHH participants; Link the AHH participants to services near potential permanent housing locations; Provide the AHH participants with the following:
a) Transportation assistance to supportive service providers;
b) Assistance in establishing public benefits such as General Relief, unemployment, SSI/SSDI, and Medi-Cal, etc..;
c) Assistance in completing applications for affordable housing (Section 8 and Public Housing); and
d) Information, resources, tools and skills to enable them to overcome health barriers, and enhance their chances of maintaining social services in the community in which they live. Create a case file for each AHH participant. All case files shall contain, but are not limited to, the following:
a) Case Management comprehensive assessment and ISP;
b) Individuals’ or households’ DHS Homeless Consent and Release Agreement form (Attachment H);
c) DHS referral forms, Consent to Release Information and any other applicable documentation or forms;
d) AHH application, rental application, Section 8 or public housing application;
e) Documentation highlighting all contacts made with or on behalf of the AHH participant;
f) Documentation of housing searches, contacts made on behalf of the client with landlords/property owners, and the outcomes of each housing referral.
Case Managers. Seven (7.0) FTE 20 The Case Manager staff shall be scheduled to provide awake supervision 21 twenty-four (24) hours per day, seven (7) days per week. The work shifts shall be 7 a.m. to 3 p.m., 22 3 p.m. to 11 p.m., and 11 p.m. to 7 a.m. 23 Duties: 24 Participate in program development. 25 Facilitate application and intake process. 26 Assist PROGRAM PARTICIPANTS through the move-in and 27 orientation process. 28 Attend weekly THP-Plus/THPP-NMD staff meeting and CJC0620 Page 41 of 46 March 17, 2020 1 community building THP-Plus/THPP-NMD participant meetings. 2 Facilitate monthly support group meetings. 3 Implement PROGRAM PARTICIPANT’s treatment plans as 4 devised by the team. 5 Support each PROGRAM PARTICIPANT in developing and 6 meeting the TILP goals.
Case Managers. The Contractor shall employ a sufficient number of case managers to meet the case management ratios listed in AMPM. The Contractor shall ensure that the Case Managers meet the administrative standards enumerated in AMPM Policy 1630.
Case Managers. Notwithstanding the provisions of articles 15:02 & 15:03, the Case Managers receive a payment of twenty (20) hours pay at their regular rate for each week of standby. It is understood that such payment is inclusive of Standby, all call-outs, wages, premiums, overtime and any Employment Standards Act requirements for Public Holidays and Overtime. There shall be no other payment for Standby and Callout duties. An employee may request to take the Standby hours in equivalent time off with pay at a time mutually agreeable to the employee and the Supervisor. A week of standby is understood as: Tuesday to Friday 4:30pm to 8:30 am Friday 4:30pm to Monday 8:30am Monday 4:30pm to Tuesday 8:30am It is understood that Standby and Callout functions will continue to be carried out by both persons within and outside the scope of this collective agreement. With this resolution for Standby & Callout for Social Housing Case Managers, the grievances 2002 – 02 (Xxxxxxxxxxx); 2002-03 (Xxxxxxx); 2002-04 (Xxxxxxxxxxx) & 2000-05 (West) are resolved on the basis of applying the above noted provisions effective from February 1, 2002.
Case Managers. Full-time case managers will provide general case management and career coaching, work with ACS Participants to create their ISS, document progress, and provide life skills support; and will support ACS Participants during transitions and the follow-up period after exit.
(1) Programs may elect to have either two (2) or three (3) full-time case managers for the entire Program.
