Enrollment and Termination of Coverage. • Enrollment The Certificate will describe the eligibility requirements for Eligible Employees (also referred to as Subscribers) and their dependents. All eligible Subscribers and their eligible dependents listed on the Group’s completed Application will be covered on the effective date of the Policy. The Insurance Company relies on the Group to provide accurate information regarding hours worked or scheduled to work exclusively for the Group.
