Equality of Coverage for Benefits Provided. The District agrees to provide paid medical, dental, vision, long term disability and life insurance benefits to nurses. With regard to medical benefits, the District’s contribution rate shall be based on the cost of the Group Health Value health insurance plan (lowest cost traditional plan), excluding Consumer Driven Health Plans (CDHP)/High Deductible Health Plans and Affordable Care Plan (ACP) that the District makes available to any District employee. The District shall continue to pay 100% of the cost of the dental plans. District shall contribute the following for the Group Health Value medical plan for 2017: Group Health Value Plan*: Employee Only: $676.06 Employee/Child: $958.03 Employee/Spouse: $1,015.44 Family: $1,379.03 *CDHP and Affordable Care plan (ACP) contributions shall be paid at the actual premium rate, if they are equal to or lower than the established rates. The District shall maintain its 2016 contribution percentage for all employees who opt to cover spouses, children and/or families. With respect to medical premiums paid by the District, the parties agree to begin negotiations over the rates listed in section 12.2 above no later than September 15, 2017 or as soon as the PEBB publishes the new rates.

With respect to medical premiums paid by the District, the parties agree to begin negotiations over the rates listed in section 12.2 above no later than September 15, 2021 or as soon as the PEBB publishes the new rates.

  • PORTABILITY OF BENEFITS The following benefits are portable:

  • Workplace Safety Insurance Benefits (WSIB) Top Up Benefits If the employee is in a class of employees that, on August 31, 2012, was entitled to use unused sick leave credits for the purpose of topping up benefits received under the Workplace Safety and Insurance Act, 1997;

  • Compensation and Employers Liability Insurance a. Statutory California Workers' Compensation coverage including broad form all-states coverage.

  • Leave of Absence for Employees Who Serve as Local Coordinators for the Ontario Nurses' Association An employee who serves as Local Coordinator for the Ontario Nurses' Association shall be granted leave of absence without pay up to a total of thirty-five (35) days annually. Leave of absence for Local Coordinators for the Ontario Nurses' Association will be separate from the Union leave provided in (a) above.

  • Eligibility for Benefits A member will not be eligible to receive Long Term Disability benefits until their Income Protection benefits have expired.

  • ’ Compensation and Employer’s Liability Coverage The Grantee shall provide workers’ compensation, in accordance with Chapter 440, F.S. and employer liability coverage with minimum limits of $100,000 per accident, $100,000 per person, and $500,000 policy aggregate. Such policies shall cover all employees engaged in any work under the Grant.

  • Termination of Coverage This Contract may be terminated as follows:

  • ’ Compensation and Employer’s Liability The policy is required only if Contractor has employees. The policy must include workers’ compensation to meet minimum requirements of the California Labor Code, and it must provide coverage for employer’s liability bodily injury at minimum limits of $1,000,000 per accident or disease.

  • Workers’ Compensation and Employer’s Liability (i) Workers’ Compensation insurance indicating compliance with any applicable labor codes, acts, Laws or statutes, state or federal, where Seller performs Work.

  • Worker's Compensation and Employer's Liability Insurance The Contractor shall have in effect during the entire life of this Agreement Workers' Compensation and Employer's Liability Insurance providing full statutory coverage. In signing this Agreement, the Contractor certifies, as required by Section 1861 of the California Labor Code, that it is aware of the provisions of Section 3700 of the California Labor Code which requires every employer to be insured against liability for Worker's Compensation or to undertake self-insurance in accordance with the provisions of the Code, and I will comply with such provisions before commencing the performance of the work of this Agreement.
