The Employer shall require an employee to furnish a standard written signed statement explaining that the employee wishes to use sick leave for the time off (Appendix B). Such a signed written statement must be submitted to the employee’s supervisor within three (3) working days of the return from such absence. 2. The Board may require a written and signed statement from the employee’s licensed physician if the sick leave is for more than three (3) consecutive days. 3. Falsification of either the employee’s signed statement or a physician’s certificate shall be grounds for disciplinary action including dismissal.
