Evidence Required for Sick Leave Usage. Upon return to work an employee shall complete an application for sick leave on the form currently in use in the automated time card system, to justify the use of sick leave. The Employer may, when an employee utilizes sick leave for medical appointments or where an absence is for three (3) consecutive days or more, require the employee to furnish a certificate from a physician, dentist, or other medical practitioner. After four (4) sick leave events within any rolling twelve (12) month period, the Employer may require the employee to provide a physician’s statement to justify the absence. Falsification of either a written signed statement or a practitioner’s certificate shall be grounds for disciplinary action including dismissal.
