The Network Service Provider will compete and sign the FFATA Certification of Executive Compensation Report Requirements form (CF 1111 or successor) if this Contract includes $25,000 or more in Federal funds (as determined over its entire term.) Federal Funding Accountability and Transparency Act. Section 2(d)(2) of the Federal Funding Accountability and Transparency Act of 2006 (Pub. L. 109-282), as amended by section 6202 of the Government Funding Transparency Act of 2008 (Pub. L. 110-252), requires SDSTA to report information on Subcontract awards. The law requires all reported information be made public, therefore, SDSTA is responsible for notifying its Subcontractors that the required information will be made public.

The purpose of the Transparency Act is to make information available online so the public can see how federal funds are spent. HCA’s Attachment 1, Federal Funding Accountability and Transparency Act Data Collection Form, attachered hereto, is considered part of this Agreement and must be completed by UW and returned along with the Agreement upon execution of this Agreement.
Federal Funding Accountability and Transparency Act a) If the Project is funded with federal funds the following shall apply: The Agency shall comply with the Federal Funding Accountability and Transparency Act of 2006 (Pub.L. 109-282), as amended by section 6202 of Public Law 000-000 ("xxx Xxxxxxxxxxxx Xxx") and the regulations and requirements of the federal government issued thereunder, including, but not limited to, 2 CFR Part 170. A1-13 Prompt Payment and Vendor Ombudsman. Pursuant to section 215.422, F.S., the Department has five (5) working days to inspect and approve goods and services, unless the bid specifications, purchase order, or this Contract specify otherwise. Any amount that is authorized for payment but is not available within forty (40) days, measured from the latter of the date a properly completed invoice is received by the Department or the goods or services are received, inspected, and approved (or within 35 days after the date eligibility for payment of a health care provider is determined), a separate interest penalty as described in section 215.422, F.S., will be due and payable in addition to the amount authorized for payment. Interest penalties less than one (1) dollar will not be paid unless the Provider requests payment. Contractor agrees to provide the information below upon request for reporting purposes. • Name of the entity receiving the award; • Amount of the award; • Information on the award including transaction type, funding agency, the North American Industry Classification System code or Catalog of Federal Domestic Assistance number (where applicable), program source; • Location of the entity receiving the award and the primary location of performance under the award, including the city, State, congressional district, and country; and an award title descriptive of the purpose of each funding action; • A unique identifier (DUNS); • The names and total compensation of the five most highly compensated officers of the entity if-- of the entity receiving the award and of the parent entity of the recipient, should the entity be owned by another entity;
