FIELD TRIP PERMISSION. Several times during the year, we have entertaining and/or instructional presenters come to school. The cost for these visitors will be divided equally among the children in attendance that day. Such fees will be added to your next billing statement. On occasion the primary classes may go on an off-site field trip, I hereby give Montessori in Motion permission to remove my child from the School for field trips. I understand that I will be given advance information about field trips before my child is removed from the school. I will have the opportunity to sign agreement or disagreement. I understand that the School will take every precaution and care to ensure my child’s safety.

Xxx 0000, Xxxxxx, XX 00000 COUNSELOR-IN-TRAINING PROGRAM APPLICATION Camp St. Xxxxxxx offers a counselor in training program designed to mold young teens into leaders. Camp St. Xxxxxxx Counselors in Training (CIT's) are a very important part of our total camp program. Camp St. Xxxxxxx CIT's are both givers and receivers of Camp St. Xxxxxxx programs. This is an opportunity for youth who have completed 7th and 8th grade to develop their leadership skills and be an active participant in Camp St. Xxxxxxx. During their week at camp, CITs participate in leadership classes, and assist with special camp projects. CIT applications are due April 1, 2016.
FIELD TRIP PERMISSION. Before a child is permitted to go on a field trip, a signed permission slip by a parent or guardian must be sent to the school. A fee to cover the cost of admission and/or bus cost may be assessed. Students who demonstrate an inability to maintain their conduct in an out-of-school environment may lose the privilege of participating in the field trip and be required to stay the day at school instead. Lost and Found A box is kept in the gym hallway for lost items such as clothing, lunchboxes, hats, gloves, etc. Items such as watches, eyeglasses and jewelry can be found in the office. Textbooks/Classroom Supplies All academic textbooks are loaned to students for their use during the school year. Students are responsible for replacing any lost or damaged textbooks or library books. Fees will be charged accordingly. Students must provide their own personal supplies such as pencils, pens, notebooks, folders, glue, erasers, etc. A supply list for individual grades is provided at the beginning of each year.
FIELD TRIP PERMISSION. I hereby give permission to the Tots N Us Children’s Academy to remove my child from the premises for any field trips, neighborhood walks, visits to the library, park, etc. by means of walking, school van or car. I understand that my child may be excluded from field trip in his/her behavior compromises their safety or the safety of the group.
