Filing a Claim with the Commission. If it is determined that the injury may result in any claim, the employee with the assistance of the supervisor, shall complete the Workers' Compensation Form 6. This form shall be completed and forwarded to the Commission within forty-eight (48) hours of the injury, subject to any extenuating circumstances. The Employer shall forward the Workers' Compensation Form 7 to the Commission within three (3) working days of the injury.
