Health and Safety Committees. Health and Safety Committees with joint Union and Management participation shall be established in each non-institutional Agency. Such committees shall have an equal number of Management and Union representatives. In each institutional Agency whose employees are covered by this Agreement, a Health and Safety Committee shall be established at each institution or facility, which Committees shall be comprised of an equal number of Union and management representatives. Pursuant to the mutual agreement of the State and all Unions certified to represent employees in any Agency or institution, the above committees may be established as multi-Union committees composed of such representatives as the State and participating Unions may mutually agree. In addition, pursuant to the mutual agreement of the State and all Unions certified to represent employees in any State Agency whose employees are covered by this Agreement, a single state- wide Health and Safety Committee, composed of such representatives as the State and participating Union may mutually agree, may be established. The committees established pursuant to the terms of this provision shall meet at mutually agreeable times, but not less frequently than once per quarter or as may be required to satisfy certification or accreditation standards. Unless extended by the mutual agreement of all members of any such committee, each meeting of the committee shall be limited to a duration of four (4) hours. Any such committee shall consider such matters relating to health and safety of employees covered by this Agreement and may make non-binding recommendations to the State, an Agency covered by this Agreement, or an institution or facility covered by this Agreement regarding such matters. Whenever an inspection of a facility is performed by another governmental Agency that relates to health and safety, a delegate shall be informed and have the right to the final written reports generated by the inspection. Every injury/occupational illness shall be investigated by the institution or Agency in a timely matter. Such investigations shall be subject to review and comment by the appropriate Health and Safety Committee.
