Integrated Care. 1. Purpose:
Integrated Care. In delivering services under the Contract, the Contractor shall develop, implement, and adhere to strategies to integrate the delivery of care across the healthcare delivery system including but not limited to, physical health, behavioral health, oral health, and long-term care services.
Integrated Care. All PIHPS will be required to have and provide upon request, signed agreements with all the Medicaid Health Plans (MHPs) in the region. The PIHPs and MHPs shall use the model coordination agreement provided in the contract as a foundational template. The Medicaid Health Plan contracts will contain the same requirement to have signed agreements with the PIHPs. Over the period of the upcoming waiver renewal cycle, new opportunities for integration with physical health care may become available in Michigan. MDCH is exploring options such as Medicaid Health Homes (ACA section 2703) and Integrated Care Dual Eligible Demonstrations (Medicare/Medicaid). Four of the new PIHP regions have been selected as the Dual Eligible Demonstration sites: Regions 1, 4, 7 and 9; others may be selected to participate in the integrated care opportunities. If approved by CMS, both the dual eligible and Medicaid Health Home opportunities will require contract amendments for PIHP regions selected to participate. The PIHPs in the Dual Eligibles regions will also require contracts with the Integrated Care Organizations in order to accomplish the Care Bridge functions and desired outcomes of integrated Medicare and Medicaid-funded behavioral health and physical health care.
Integrated Care a) The parties have undertaken several initiatives aimed at providing sustainable high quality care. These initiatives include Primary Care Networks, payment reforms, and improvements to information management and technology. b) The parties acknowledge that increased attention must be made to integrated care throughout the health care system, which requires the integration of delivery models. Towards that end, the parties agree to create an Integrated Care Consultation Agreement pursuant to section 4 of the AMA Agreement.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons.   • Verizon OSS Services 8.2.1 Upon request by PNG, Verizon shall provide to PNG Verizon OSS Services. Such Verizon OSS Services will be provided in accordance with, but only to the extent required by, Applicable Law. 8.2.2 Subject to the requirements of Applicable Law, Verizon Operations Support Systems, Verizon Operations Support Systems functions, Verizon OSS Facilities, Verizon OSS Information, and the Verizon OSS Services that will be offered by Verizon, shall be as determined by Verizon. Subject to the requirements of Applicable Law, Verizon shall have the right to change Verizon Operations Support Systems, Verizon Operations Support Systems functions, Verizon OSS Facilities, Verizon OSS Information, and the Verizon OSS Services, from time-to-time, without the consent of PNG. 8.2.3 To the extent required by Applicable Law, in providing Verizon OSS Services to PNG, Verizon will comply with Verizon’s applicable OSS Change Management Guidelines, as such Guidelines are modified from time-to-time, including, but not limited to, the provisions of the Guidelines related to furnishing notice of changes in Verizon OSS Services. Verizon’s OSS Change Management Guidelines will be set out on a Verizon website.

  • Telemedicine Services This plan covers clinically appropriate telemedicine services when the service is provided via remote access through an on-line service or other interactive audio and video telecommunications system in accordance with R.I. General Law § 27-81-1. Clinically appropriate telemedicine services may be obtained from a network provider, and from our designated telemedicine service provider. When you seek telemedicine services from our designated telemedicine service provider, the amount you pay is listed in the Summary of Medical Benefits. When you receive a covered healthcare service from a network provider via remote access, the amount you pay depends on the covered healthcare service you receive, as indicated in the Summary of Medical Benefits.
