Investigation of Accidents and Injuries. ‌ The OHS Committee shall be notified in a timely manner of each accident and injury involving an employee which has occurred since the last meeting of the OHS Committee. Incidents will be jointly investigated, by one representative of the Union and one employer representative who will report to the Union and the Employer on the nature and cause of the accident or injury. Where the OHS Committee makes a report, the OHS Committee shall decide on the format of the report and whether the report should be sent to the Workers' Compensation Board. In the event of a fatality, the Employer shall immediately notify the President of the Union or his/her designate.

  • Accident Investigations When an accident occurs involving the equipment or personnel of a Supporting Agency, the Protecting Agency shall immediately notify the jurisdictional and supporting agencies. As soon as practical, the Protecting Agency shall initiate an investigation of the accident.
