Group Life Insurance and Accidental Death and Dismemberment at the following amounts: Oct.1/14 Oct. 1/15 Life Insurance $54,000 $54,000 A.D. and D. $54,000 $54,000 Dependent life insurance to provide a $7,000 benefit for the spouse and a $3,000 benefit for each dependent child. Employees retiring between the age of fifty-five (55) and sixty-two (62) years with twenty (20) years of service - $10,000 in Life Insurance. Employees retiring between the age sixty-two (62) years and sixty-five (65) years with a minimum twenty (20) of service - $5,000 in Life Insurance. Employees retiring at age sixty-five (65) with a minimum twelve (12) years of service - $5,000 in Life Insurance.
Group Life Insurance and Accidental Death. Dismemberment (AD&D) - 100% coverage for one times (1.0x) base annual earnings for life insurance and AD&D equal to the group life insurance amount.
