Group Life Insurance and Accidental Death and Dismemberment (AD&D) (a) Permanent and probationary Employees shall join the group life insurance plan on the first day of the calendar month following six (6) months of employment. (b) Coverage is based on the following general principles: (i) Two (2) times annual salary, rounded to the nearest thousand dollar to a maximum of $200,000.00, or as set out in the master policy, whichever is greater. (ii) Accidental death and dismemberment coverage for loss within three hundred and sixty-five (365) days of an accident of life, limb or sight according to the schedule set forth in the master policy. (c) The Employer shall pay 100% of the premium cost.

Employees retiring at age sixty-five (65) with a minimum twelve (12) years of service - $5,000 in Life Insurance.
Group Life Insurance and Accidental Death and Dismemberment Effective April 1, 2001 for Employees earning more than $12,000 annually. Effective April 1, 2002 for Employees earning more than $12,000 annually. Effective April 1, 2003 for Employees earning more than $13,222 annually. The Agency shall pay one hundred percent (100%) of the monthly premium of Group Life Insurance plus Accidental Death and Dismemberment Insurance on behalf of each of these Employees in order to achieve a benefit level of three times (3X) the annual gross earnings.

Group Life Insurance and Accidental Death and Dismemberment Effective October 1, 1999: Effective October 1, 2000 (1) The Employer will provide a group life insurance plan. (2) The plan shall provide basic life insurance in the amount of $25,000.00 (effective October 1, 2002: $50,000.00) and standard 24 hour accidental death and dismemberment insurance until age sixty-five (65). After the age of sixty-five (65) the amount of coverage shall decrease to $12,500.00 (effective October 1, 2002: $25,000.00) until the age of seventy (70), at which time the group insurance coverage will cease. On termination of employment (excluding retirement) coverage shall continue without premium payment for a period of thirty-one (31) days, during which time the conversion privilege may be exercised; that is, the individual covered may convert all or part of his/her group life insurance into any whole life, endowment or term life policy normally issued by the insurer and the insurer’s standard rates at the time, without medical evidence. (3) The Employer shall pay 100% of the premium.
Group Life Insurance and Accidental Death and Dismemberment Insurance The Employer will contribute, effective May 1, 2000, for full-time employees enrolled in the Group Life and Accidental Death and Dismemberment Insurance Plan, $6.80 per month towards the premium cost for $25,000 of life and accidental death and dismemberment insurance.
