Leave Time Associated With Sick Leave. Leave forms for absences due to illness/injury or emergency situations as specified in Subsection (a), above, shall be completed by the employee as soon as possible. Medical certification for absences of five (5) days or more for absences due to medical reasons is required if requested by the Employer. Medical certification may also be required, when good cause exists to believe the employee is abusing sick leave, for absences less than five (5) days. Sick leave shall be requested on the payroll timesheet.
