Life Insurance/Disability Insurance. The Company shall provide Employee with such life and/or disability insurance as the Company may from time to time make available to senior executives of the Company.

During the Employment Period, the Company shall keep in force and pay the premiums on the split-dollar life insurance policy referenced in the Split Dollar Insurance Agreement between Avalon and Executive, subject to reimbursement by Executive as provided in such Split Dollar Insurance Agreement. The Company shall reimburse Executive for the cost of the comprehensive disability insurance policy, which is in effect on January 1, 1997, and shall also be responsible for any increases in premiums which become effective during the Employment Period as may be necessary to maintain the same level of insurance as in effect on January 1, 1997. Executive agrees to submit to such medical examinations as may be required in order to maintain such policies of insurance.

Life Insurance/Disability Insurance. The Company, at its expense, will purchase life insurance on your life in the face amount of not less than $1 million with a beneficiary designated by you. The Company will also arrange and pay the premiums associated with short and long-term disability insurance on your behalf with annual benefits in an amount equal to 60% of your Base Salary. During the time period during which you receive benefits under the short and long-term disability policy(-ies), the Company shall not be obligated to pay your Base Salary.

During the Employment Period, the Company shall, to the extent reasonably available on customary terms and rates, (i) provide Employee with term life insurance in a face amount equal to $1,000,000, and (ii) have Employee covered by reasonably comprehensive disability insurance or, at Employee's election, otherwise reimburse Employee for the cost of such a policy in an amount not to exceed $5,000 per year; provided that such $5,000 amount shall be increased as the age of the Employee increases for any year during the Employment Period as may be necessary to maintain the same level of insurance as in effect during the first year of the Employment Period. Employee agrees to submit to such medical examinations as may be required in order to secure or maintain such policies of insurance.

As of the Effective Date, during the Employment Period, Executive will receive a split dollar life insurance agreement and comprehensive disability policy comparable to those provided to comparable Avalon executives. Such life insurance amount shall equal $750,000, and both the life insurance and disability policy shall be subject to evidence of Executive's insurability. Executive agrees to submit to such medical examinations as may be required in order to establish or maintain such policies of insurance.

Beginning on the first anniversary of the Effective Date and for the balance of the Employment Period, the Company shall, to the extent reasonably available on customary terms and rates, (i) provide Employee with term life insurance in a face amount equal to $1,000,000, and (ii) have Employee covered by reasonably comprehensive disability insurance or, at Employee's election, otherwise reimburse Employee for the cost of such a policy in an amount not to exceed $5,000 per year; provided that such $5,000 amount shall be increased as the age of the Employee increases for any year during the Employment Period as may be necessary to maintain the same level of insurance as in effect during the second year of the Employment Period. Employee agrees to submit to such medical examinations as may be required in order to secure or maintain such policies of insurance.
Life Insurance/Disability Insurance. A. The District shall pay the full cost of $50,000 of group term life insurance for each eligible employee. All employees shall receive such life insurance coverage on the first day of the calendar month following completion of six (6) months service.
Life Insurance/Disability Insurance. SECTION 1. After completion of the required probationary period, each officer shall be eligible for a ten thousand ($10,000.00) dollar life insurance policy payable to a beneficiary of the employee's choice.
