Common use of Long Form Evaluation Clause in Contracts

Long Form Evaluation. Observations: At least one of the required observations must be preplanned and scheduled with the Employee. Criteria addressed must be those noted in Section 8.7 or Section 8.8 as appropriate. ESA observations may include a combination of pre- and post-clinical discussions and/or actual observations. All observations shall be documented in writing within five days, with a copy transmitted to the Employee within five days of the date the observation report is prepared. Each Employee shall have the opportunity for a minimum of two confidential conferences during each school year for the purpose of evaluation.
