Monitoring of Service Provision. 10.1 The Provider acknowledges the right of the Authority to monitor the provision made for the care and education of a Learner specified in Schedule 2 (Individual Placement Agreement) placed by the Authority. This will include monitoring all aspects of the provision provided, irrespective of the Element (Element 1, Element 2 or Element 3) it is delivered through. Monitoring may include visits by the Authority's representatives or agents by prior agreement with the Head-teacher, or unannounced visits consistent with the Duties provided that upon any such visit taking place the number of visitors at any one time should be no more than necessary for statutory/inspection purposes. Every effort shall be made to ensure the continued privacy of Learners and minimal disruption to the education of Learners.
10.2 Such arrangements shall be in addition to and not prejudice any Statutory Inspection under provision of the appropriate Acts.

Monitoring of Service Provision. Each year, a program report card will be reviewed by the Transition Unit. Any provider who does not meet the above criteria will be further monitored. Failure to meet program outcomes may result in the development of a corrective action plan or the termination of this agreement. The provider will cooperate with all OVR monitoring activities and maintain documentation of service delivery. OVR reserves the right to monitor all activities and documentation associated with this Provider Agreement at the completion of year one (1). This may include review by:
1. scheduled and unscheduled onsite visits;
2. solicitation of school, student, worksite and OVR staff feedback;
3. provider report card based on CWDS data;
4. review of required documentation;
5. evaluation of performance outcomes or 6. auditing of financial statements.

Monitoring of Service Provision. The provider will cooperate with all OVR monitoring activities and maintain documentation of service delivery. Upon completion of year one, the OVR Central Office Specialist and District Administrator will review the Provider Outcome and Expenditure Report and OVR staff questionnaires. The OVR Central Office Specialist will schedule an on-site visit to review services and documentation, if technical assistance is needed. After the fourth year, the OVR Central Office Specialist, Contract & Grantee Analyst and District Administrator will assess services and review provider report card. A service evaluation questionnaire will be completed. An on-site monitoring will be scheduled to review information and determine the potential for renewal of the agreement. If agreed upon by all parties, facilitation of the renewal process will occur during the fourth year of the agreement. An on-site monitoring visit to review services will be completed before the five-year expiration date.
