Notice of Replies to Reports. 1. All observations, evaluations, and conference reports shall be signed by the teacher to signify the teacher's having seen and read it prior to placement in the teacher's personnel file. No such reports shall be maintained unless they are disclosed to the teacher and included in said teacher's personnel file.
