Optional Accidental Death Sample Clauses

Optional Accidental Death and Dismemberment Insurance of up to $250,000 for the employee, up to half of the employee's insured amount for the employee's spouse and up to $250,000 for family coverage, all of which can be obtained in $5,000 increments. The Employee shall pay 100% of premium cost. Note: A more detailed summary of the above coverage is available through the Benefits Division of Human Resources.
