Group Life and Accidental Death and Dismemberment. The District shall provide permanent unit members a fully paid Group Life and Accidental Dismemberment Policy providing a $25,000 level term insurance coverage to retirement or separation from employment, plus a $25,000 accidental death benefit.
Group Life and Accidental Death and Dismemberment. Effective April 1, 2000 Eligible regular employees shall be provided with Group Life and Accident Coverage. The Plan shall provide ten thousand dollars ($10,000.00) coverage, until the age of sixty-five (65), and shall include accidental death and dismemberment coverage. After the age of sixty-five (65), the amount of coverage shall decrease to five thousand dollars ($5,000.00) until the age of seventy (70), at which time the group insurance coverage will cease. The Employer agrees to make available optional voluntary life insurance (maximum $200,000) for eligible faculty members and their spouse/common-law spouse, subject to the applicant meeting insurance company requirements. All premiums for this optional life insurance will be paid by the faculty member. Life insurance benefits will be paid in accordance with the schedule of benefits listed in the insurance carrier's plan and are subject to the limitations specified in the plan including eligibility requirements.
Group Life and Accidental Death and Dismemberment. The Employer will pay 100% of the premiums for the group life and accidental death and dismemberment insurance plans. The plan will provide basic life insurance in the amount of $50,000 and standard 24-hour accidental death and dismemberment insurance until age 65. At the age of 65 the amount of coverage will decrease to $25,000 until the age of 70, at which time the group insurance coverage will cease. Employees may purchase additional insurance provided this option is available by the carrier. The Employer will deduct the appropriate amount from the employee's pay for this option. On termination of employment (excluding retirement) coverage for group life will continue without premium payment for a period of 31 days during which time the conversion privilege may be exercised; that is, the individual covered may convert all or part of their group life insurance into any whole life, endowment or term life policy normally issued by the insurer and the insurer's standard rates at the time, without medical evidence. Employees will be entitled to advance payment of Group Life Benefits in accordance with Memorandum of Agreement #7 (Advance Payment of Group Life Benefits).

Group Life and Accidental Death and Dismemberment. For all eligible Employees, the Employer shall pay one hundred percent (100%) of the premium costs of a group life insurance policy and accidental death and dismemberment insurance. The group life policy shall provide for the payment of three (3) times annual salary on the death of an Employee. The accidental death and dismemberment insurance shall provide for an additional payment of three (3) times annual salary on the accidental death of an Employee and coverage for accidental dismemberment. Participation of eligible Employees shall be a condition of employment. Each continuing Employee is eligible to participate in the group life insurance plan. When successive full-time term appointments result in a term of uninterrupted employment of four (4) months or more, term Employees shall be eligible for group life and accidental dismemberment benefits from the date of the beginning of the contract which results in the total period of employment of four (4) months or more.
