Common use of Optional Group Life Clause in Contracts

Optional Group Life. Subject to the provisions of the Plan, eligible employees shall be entitled to purchase optional Group Life Insurance coverage in units of ten thousand dollars ($10,000) up to a maximum of two hundred and fifty thousand dollars ($250,000). The employee shall pay one hundred percent (100%) of the premiums for the optional coverage.

Optional Group Life. Insurance Subject to the provisions of the Plan, eligible employees shall be entitled to purchase optional Group Life Insurance coverage in units of ten thousand dollars ($10,000) 5,000, from a minimum of $10,000 up to a maximum of two hundred $500,000 for themselves and/or an eligible spouse, and fifty thousand dollars (for a dependent child in the flat amount of $250,000)5,000. The employee shall pay one hundred percent (100%) % of the premiums for the optional coverage.

Subject Effective as soon as possible following August subject to the provisions of the Plan, eligible employees shall be entitled to purchase optional Group Life Insurance coverage in units of ten thousand dollars ($10,000) up to a maximum of two hundred and fifty thousand dollars ($250,000).
