PARTICIPANT Enrollment Information. Description The CONTRACTOR must provide the following information, at a minimum, to PARTICIPANTS upon enrollment: Information about accessing online account and mobile app. Directions on updating profile information, view account balance(s), check claim status, set-up direct deposit, substantiate a claim, request a reimbursement, request a DEBIT CARD. Information about how to properly manage account and PARTICIPANT responsibilities. The CONTRACTOR’S contact information, including the dedicated toll-free customer service phone number, business hours, and website address. (See Section 130C Participant Information.)
