Personal Leave - Personal Illness. A. A request (using the Extended Leave of Absence form) for leave of absence without pay for illness must be accompanied by a statement from the attending physician indicating that the member cannot perform the duties of his/her position. Before returning to teaching from such leave, the Teacher shall present a statement from the attending physician that he/she is able to fulfill his/her regular duties with the Employer. No experience credit shall be granted for a leave of absence for illness on the salary schedule.
