Physical Fitness Certification. In accordance with 105 ILCS 5/24-5, all individuals performing Services who provide services to students or in schools, or any other individuals subject to the criminal background check requirements in 105 ILCS 10-21.9, shall submit (within ninety (90) days prior to commencing the provision of Services under this Agreement) to a physical examination by a physician licensed in Illinois or any other state to practice medicine and surgery in all its branches, a licensed advanced practice registered nurse, or a licensed physician assistant, and shall provide the District with evidence of physical fitness to perform duties assigned and freedom from communicable disease. The cost of this examination shall be paid by the Provider. A certification from Provider to the District that the individuals providing services have provided the required physical fitness certification to Provider will be sufficient unless the District requires, or notifies the Provider that the Illinois State Board of Education is requiring, that the actual medical certification be provided to the District.

Physical Fitness Certification. In accordance with 105 ILCS 5/24-5, all individuals performing Educational Teaching Services who provide services to students or in schools, or any other individuals subject to the criminal background check requirements in 105 ILCS 10-21.9, shall submit (within ninety (90) days prior to commencing the provision of Educational Teaching Services under this Agreement) to a physical examination by a physician licensed in Illinois or any other state to practice medicine and surgery in all its branches, a licensed advanced practice registered nurse, or a licensed physician assistant, and shall provide the District with evidence of physical fitness to perform duties assigned and freedom from communicable disease. The cost of this examination shall be paid by the Provider. A certification from Provider to the District that the individuals providing services have provided the required physical fitness certification to Provider will be sufficient unless the District requires, or notifies the Provider that the Illinois State Board of Education is requiring, that the actual medical certification be provided to the District.
Physical Fitness Certification. Evidence of a physical exam and a negative patch or Tuberculosis test (documentation must include date of exam/test performed by a licensed physician not more than ninety (90) days prior to the commencement of services with the District).However, in lieu of providing the foregoing evidence directly to the District, a certification from Provider to the District that the individuals providing services have provided the required physical fitness certification to Provider will be sufficient unless the District notifies the Provider that the Illinois State Board of Education is requiring that the actual medical certification be provided to the District. This certification may be provided to the District by the Provider sending the following email statement to the Director of Student Services: “I hereby certify that [name of Interpreter] has provided to SLII evidence of a physical exam and negative patch or TB test performed by a licensed physician not more than ninety (90) days prior to the commencement of the Assignment.”
Physical Fitness Certification. In accordance with 105 Il-CS 5/24-5, any individuals performing services hereunder who will have direct, daily contact with students shall submit to a physical examination by a physician licensed in Illinois or any other state to practice medicine and surgery in all its branches and shall provide the SCHOOL with evidence of physical fitness to perform duties assigned and freedom from communicable disease. The cost of this examination shall be paid by AVEANNA.

  • CHILD SUPPORT CERTIFICATION Under Section 231.006, Texas Family Code, the Engineer certifies that the individual or business entity named in this contract, bid, or application is not ineligible to receive the specified grant, loan, or payment and acknowledges that this contract may be terminated and payment may be withheld if this certification is inaccurate. If the above certification is shown to be false, the Engineer is liable to the state for attorney’s fees, the cost necessary to complete the contract, including the cost of advertising and awarding a second contract, and any other damages provided by law or the contract. A child support obligor or business entity ineligible to receive payments because of a payment delinquency of more than thirty (30) days remains ineligible until: all arrearages have been paid; the obligor is in compliance with a written repayment agreement or court order as to any existing delinquency; or the court of continuing jurisdiction over the child support order has granted the obligor an exemption from Subsection (a) of Section 231.006, Texas Family Code, as part of a court- supervised effort to improve earnings and child support payments.

  • CERTIFICATION CLAUSES The CONTRACTOR CERTIFICATION CLAUSES contained in the document CCC 04/2017 are hereby incorporated by reference and made a part of this Agreement by this reference as if attached hereto.

  • Drug-Free Workplace Certification As required by Executive Order No. 90-5 dated April 12, 1990, issued by the Governor of Indiana, the Company hereby covenants and agrees to make a good faith effort to provide and maintain a drug-free workplace at the Project Location. The Company will give written notice to the IEDC within ten (10) days after receiving actual notice that the Company, or an employee of the Company in the State of Indiana, has been convicted of a criminal drug violation occurring in the workplace. False certification or violation of this certification may result in sanctions including, but not limited to, suspension of payments under the Agreement, termination of the Agreement and/or debarment of contracting opportunities with the State for up to three (3) years. In addition to the provisions of the above paragraph, if the total amount set forth in the Agreement is in excess of $25,000.00, the Company agrees that it will provide a drug-free workplace by: A. Publishing and providing to all of its employees a statement notifying them that the unlawful manufacture, distribution, dispensing, possession or use of a controlled substance is prohibited in the Company’s workplace, and specifying the actions that will be taken against employees for violations of such prohibition;
